Registered nurses who work with The Futures HealthCore provide both consultation and direct services to a diverse healthcare population. They act as coordinators of care, educators, care managers, advocates and as facilitators of health and wellness.

Their role is to:

• Assess individuals based on physician referral and provide recommendations and feedback to appropriate personnel
• Coordinate components of required care
• Report progress, irregularities and/or changes in status to the physician and other team members as appropriate
• Contribute to the development and implementation of health-related goals for referred individuals
• Document treatment activity, observations, and treatments effectiveness as required
• Ensure that any nursing delegation processes are implemented appropriately, as per organization policy and in compliance with the needs of referred individuals
• Maintain patient confidentiality

Individuals are referred to the nursing staff at Futures when they experience a need for:

Diabetes Management Heart Disease Management Respiratory management Wound Management Skin care Tube feedings Catheterizations Dressing changes Infection Control
